INSURANCE & PAYMENT INFORMATION
We are happy to file your insurance claim for you.
Our contracts with the insurance company require that we collect any deductibles and or co-insurance at the time of service. Please ask a staff member if you need help with your insurance information.
We are a provider for most dental and medical insurance plans and will do our best to assist you with your insurance plan/benefits.
Regardless of our efforts to be as accurate as possible, there is always a possibility for discrepancy which could vary from complete denial to 100% coverage. Our job is to relate the information given to us by your insurance company so that you are aware of your potential out-of-pocket expenses.
